North Bay Physician LEG Membership
The following is a list of requirements for North Bay Physicians Local Education Group membership
Affiliations
- Hold current registration with the CPSO in good standing
- Hold current malpractice protection with the CMPA for the province of Ontario
- Hold NOSM faculty appointment in good standing
- Hold NBRHC professionals staff appointment in good standing
- Cannot hold membership with any other Local Education Group (i.e. North Bay Pediatric LEG or WNGH LEG)
Responsibilities
- Must be willing to teach NOSM University learner rotations / blocks / assignments. Teaching may include preceptoring, lecturing, workshops, or formalized mentoring.
Documentation
- Completion of a NOAMA Declaration & Consent (D&C) Form
- Completion of NBPLEG Schedule B Associates Subscription Declaration Form
- Completion of NBPLEG Payment Form & Direct Deposit Form
Location
- Primary practice in located in North Bay and area, which is defined by North West to North Bay, South to Powassan and East to Astorville (see map below)
